查看更多>>摘要:Mandibular condylar fractures are among the most common facial fractures and some of the most difficult to manage.Opinions about the management of mandibular condylar fractures differ among surgeons.With the implementation of new technology,an increased understanding of fracture management,and better functional and morphological outcomes reported in the literature,open reduction and internal fixation is becoming many surgeons' preferred choice for the treatment of condylar fractures.Because surgical treatment of such fractures is complex,certain factors must be considered to achieve satisfactory outcomes.In this article,we summarise six key points in the management of mandibular condylar fractures:virtual evaluation of condylar fracture,a suitable surgical approach,good reduction,stable internal fixation,repair of the articular disc,and restoration of the mandibular arch width.We believe that these points will help to improve the prognosis of mandibular condyle fractures.
查看更多>>摘要:Purpose:Given the increased risk of accidents in patients with attention deficit and hyperactivity disorder (ADHD) or maternal anxiety/depression,we aimed to investigate the frequency of the two diseases in children with penetrating eye injury (PEI).Methods:Altogether 79 children,39 with PEIs and 40 healthy individuals (control group),aged 5-15 years,underwent a complete ophthalmologic examination.Afterwards,schedule for affective disorders and schizophrenia for school-aged children was conducted to assess the psychiatric diagnosis of all children.Turgay diagnostic and statistical manual of mental disorders (DSM-IV)-based child and adolescent behavior disorders screening and rating scale (T-DSM-IV-S) was filled by parents to evaluate the severity of ADHD symptoms.The depression and anxiety levels of mothers of each group were evaluated by two self-report measures:the Beck depression scale and the state-trait anxiety inventory (STAI),respectively.Data were analyzed by IBM SPSS version 22.0.The Chi-square and Fisher's exact test were used to determine whether there is a significant difference between qualitative variables while independent sample t and Mann-Whitney U tests to compare quantitative variables.Results:The only diagnostic difference was a significantly higher frequency of ADHD among patients with PEIs (48.7% in PEI vs.17.5% in control group,x2 =7.359,p =0.007).The total scores of the T-DSM-IV-S (attention subscale U =418.000,p =0.006;hyperactivity subscale U =472.000,p =0.022) and maternal state-trait anxiety inventory (maternal STAI-state U =243.000,p =0.003;maternal STAI-trait U =298.000,p =0.021) were significantly higher in the PEI group than in control group.In logistic regression,children with PEI had a tendency to have a 3.5-fold increased risk for ADHD (OR =3.538,CI =0.960-13.039,p =0.058).Conclusion:ADHD was detected almost 1 in 2 children with PEIs.Besides,the maternal anxiety level was significantly higher in the PEI group than in the control group.This association should be further explored via a future prospective longitudinal study.Since a proper treatment of ADHD in children and anxiety treatment in mothers may prevent vision loss following PEIs in children.
查看更多>>摘要:Trauma during pregnancy deserves special attention because of its management objectives,i.e.wellbeing of both pregnant woman and foetus.Maxillofacial trauma directly affects the nutrition of foetus by interfering with the normal functions in a pregnant woman such as mouth opening,mastication and breathing.Hence early restitution of form and function of maxillofacial skeleton is essential.However,the gravid status is associated with numerous anatomical and physiological changes which present with clinical dilemma related to imaging and treatment.A careful scrutiny of the patient's systemic and gestational status is absolutely essential before,during and after instituting any interventional procedures.We present a case of bilateral condyle fracture in a 30-year-old pregnant woman in the third trimester (32 weeks).She was treated with inter maxillary fixation using orthodontic brackets & elastics.After successful restitution of occlusion,the patient was advised aggressive physiotherapy which ensured normal mouth opening.Two weeks later,the patient delivered uneventfully.The patient was followed up at one month and 3 month and demonstrated restitution of normal occlusion,mouth opening and lower facial height.This article aims at analyzing the contemporary principles in management of maxillofacial trauma in a pregnant woman and clarifying the common misconceptions.
查看更多>>摘要:Penetrating head injury is rare,and thus management of such injuries is non-standard.Early diagnosis and intraoperative comprehensive exploration are necessary considering the complexity and severity of the trauma.However,because of the lack of microsurgical techniques in local hospitals,the possible retained foreign bodies and other postoperative complications such as cerebrospinal fluid (CSF) leak usually require a rational design for a secondary operation to deal with.We present a case of a 15-year-old boy who was stabbed with a bamboo stick in his left eye.The chopsticks passed through the orbit roof and penetrated the skull base.In subsequent days,the patient sustained CSF leak and intracranial infection after an unsatisfied primary treatment in the local hospital and had to request a secondary operation in our department.Computed tomography including plain scan,three dimension reconstruction and computed tomographic angiography are used to determine the course and extent of head injury.A frontal craniotomy was performed.Three pieces of stick were found residual and removed with the comminuted orbit bone fragments.A pedicled temporalis muscle fascia graft was applied to repair the frontier skull base and a free temporalis muscle flap to seal the frontal sinus defect.Aggressive broadspectrum antibiotics of vancomycin and meropenem were administrated for persistent fever after operation.CSF external drainage system continued for 12 days,and was removed 10 days after temperature returned to normal.The Glasgow coma scale score was improved to 15 at postoperative day 7 and the patient was discharged at day 22 uneventfully.We believe that appropriate preoperative surgical plan and thorough surgical exploration by microsurgery is essential for attaining a favorable outcome,especially in secondary operation.Good postoperative recovery depends on successfully management before and after operation for possible complications as well.
查看更多>>摘要:Pericyte,a kind of pluripotent cell,may regulate the irrigation flow and permeability of microcirculation.Pericytes are similar to the smooth muscle cells,which express several kinds of contractile proteins and have contractility.The dysfunction of pericytes is related to many microvascular diseases,including hypoxia,hypertension,diabetic retinopathy,fibrosis,inflammation,Alzheimer's disease,multiple sclerosis,and tumor formation.For a long time,their existence and function have been neglected.The distribution,structure,biomarker,related signaling pathways as well as the roles of pericytes on vascular diseases will be introduced in this review.
查看更多>>摘要:With the deepening of research,proteomics has developed into a science covering the study of all the structural and functional characteristics of proteins and the dynamic change rules.The essence of various biological activities is revealed from the perspectives of the biological structure,functional activity and corresponding regulatory mechanism of proteins by proteomics.Among them,phospholipid-binding protein is one of the hotspots of proteomics,especially annexin A1,which is widely present in various tissues and cells of the body.It has the capability of binding to phospholipid membranes reversibly in a calcium ion dependent manner.In order to provide possible research ideas for researchers,who are interested in this protein,the biological effects of annexin A1,such as inflammatory regulation,cell signal transduction,cell proliferation,differentiation and apoptosis are described in this paper.
查看更多>>摘要:Purpose:Recurrent dislocation of shoulder (RDS) is a common injury in high demand professionals,like athletes and military personnel.The treatment for the patients with Bankart lesion is the arthroscopic repair.This present study compares the outcomes of two different techniques of arthroscopic Bankart repair i.e.a standard two anterior portals technique and a single anterior portal technique in patients with RDS.Methods:Patients with traumatic RDS met the inclusion criteria were managed with Bankart repair using either two anterior portals (Group A) or a single anterior portal (Group B) technique.Patients were evaluated before the intervention and at the mean follow-up of approximately two years using Rowe score,Oxford shoulder score and Tegner activity scale.Results:The mean age of the patients in Groups A (n =34) and B (n =37) was 29.64 years and 29.05 years respectively (p =0.66).The dominant shoulder was involved in 27 patients in Group A and 22 patients in Group B (p =0.069).The operative time in Group A and B was 68.52 min and 46.35 min,respectively (p < 0.001).The complications at follow-up,the mean Rowe score and Oxford score improved significantly in both groups compared with the pre-operative values.However,the final outcome scores were not significantly different between the both groups.The median Tegner's score preoperatively and at follow-up was 7 and 6,respectively in Groups A and B.Conclusions:Single anterior portal technique is an effective treatment modality,yielding a similar outcome as two anterior portals technique in the management of RDS.
查看更多>>摘要:Purpose:Severe damage to the femoral head in patients with osteonecrosis has a high impact on morbidity.Despite early diagnosis,the treatment outcome is still unsatisfactory.This study aimed to explore the expression of vascular endothelial growth factor (VEGF) and cyclic guanine monophosphate (cGMP) serum level as the risk factors of femoral head osteonecrosis in alcohol-exposed Wistar rats.Methods:This was an experimental study using randomized post-test only control group design,with samples using 10-14 weeks Wistar male rats.Rats were then divided into 6 groups:3 groups without intervention,and 3 groups with intervention using 40% alcohol given perorally.Each one group from intervention and control group was euthanized by the end of the week for 3 consecutive weeks.Proximal femurs were examined under microscope for osteonecrosis,immunohistochemically for VEGF,and blood serum for cGMP levels.Results:VEGF expression in the femoral head of alcohol-exposed Wistar rats was lower than those not exposed to alcohol (p < 0.005).Blood serum cGMP levels of alcohol-exposed Wistar rats were higher than those not exposed to alcohol (p < 0.005).The number of necrotic osteocytes in the femoral head of Wistar rats exposed to alcohol was greater than those not exposed to alcohol (p < 0.005).There are significant differences between VEGF,cGMP levels,and number of necrotic osteocytes in the control group and treatment at 1st,2nd and 3rd week (p < 0.005).Conclusions:Based on the result of this study,VEGF and cGMP may be considered as diagnostic biomarkers for alcohol-induced femoral head osteonecrosis.
查看更多>>摘要:Purpose:Comparison of effective road safety approaches with those of relatively similar countries can be used to identify possibilities for safety improvement.Since there is no clear and comprehensive study of countries' current and successful approaches to road safety in the world,the aim of this study was to identifying common road safety approaches in the world.Methods:This study was performed using scoping review and thematic analysis.The study followed the approach proposed by Arksey and O'Malley.In this study all articles were selected without time limit by searching in the following databases:Web of Science,PubMed,Scopus,ProQuest,and Embase.An initial search of 5612 papers was found and finally,20 papers met the inclusion criteria and were analyzed.Results:There were different road safety approaches in different countries around the world,which were classified in three themes:traditional approach,systemic approach,and vision zero.The traditional approach includes the sub-theme of the road-user approach,and the causal approach.The systemic approach also includes sub-themes of sustainable safety,safety system,and the United Nations plan for decade of action.Conclusion:A systemic approach to road safety seems to be welcomed by most developed and developing countries,and a paradigm shift towards a safe system has taken place.Also,given the successful results of implementing vision zero in leading countries,most countries are trying to design and implement this approach.Finally,the choice and implementation of road safety approaches varies according to the principles,priorities and infrastructure of each country.
查看更多>>摘要:Extradural hematoma (EDH) is a rare but serious complication of brain injury.The supra-and infratentorial EDH is even rarer and only a few articles focusing on this topic have been published.The clinical manifestations are nonspecific but early diagnosis of supra-and infra-tentorial EDH and prompt treatment are mandatory to avoid complications.We report this case of a supra-and infra-tentorial subacute EDH on a 4 years old child.
