    阴道再造的治疗进展

    查看更多>>摘要:阴道缺损是女性生殖道畸形的表现之一,可因先天发育异常或会阴部肿瘤、外伤、产伤等后天因素造成。对于跨性别女性患者,则是在心理上缺失阴道。阴道再造应用于阴道功能不完善的女性及要求成为女性的患者,主要有非手术和手术治疗2种方法。近年来,随着医疗技术的进步,阴道再造术有了进一步的发展。该文对于阴道再造的历史、非手术和手术治疗方法的进展进行综述。 Vaginal agenesis is one of the manifestations of malformations of the female genital tract, which can be caused by congenital developmental abnormalities, or by perineal tumour, trauma or birth injuries. In transgender female patients, there is a psychological absence of the vagina. Vaginal reconstruction is used in women with a dysfunctional vagina and in patients who require to become women, and can be divided into non-surgical and surgical treatment. In recent years, vaginal reconstruction has been further developed with advances in medical technology. This article provides an overview of the history of vaginal rejuvenation and the advances in non-surgical and surgical treatment method .

    查看更多>>摘要:使用聚丙烯酰胺水凝胶(PAHG)进行隆乳手术曾在中国十分流行。该文报道了1例PAHG隆乳20年后,无其他诱因于2019新型冠状病毒(2019-nCoV)感染后出现单侧乳房迟发型炎症反应的患者。患者女,45岁,在2019-nCoV感染3周后出现右侧乳房明显红肿热痛,范围上至锁骨、下至上腹部,考虑诊断为病毒感染伴发乳房迟发型炎症反应。急诊手术术中发现PAHG呈黄绿色、稀糊状涌出,腐臭味明显,存在大量组织坏死糜烂及脓液,创面广泛弥漫性渗血,术中血红蛋白下降明显。术后无并发症,于术后1周出院。2019-nCoV对人体异物及免疫系统的影响还有很多未知区域,可能会引起凶险的迟发型炎症反应,如该例中的电解质紊乱、大量失血及可能发生的感染性休克,在临床中需引起足够重视、给予及时干预。报道各种可能出现的症状和机制有利于提高整形外科医生的认识水平、加强对填充材料使用的谨慎程度。 Breast augmentation using polyacrylamide hydrogel (PAHG) was widely used in China. We present the first case of delayed breast inflammation after 2019 novel coronavirus(2019-nCoV) infection in a patient who recieved breast augmentation with PAHG 20 years ago, without any other predispositions. The 45-year-old female patient complained of severe breast swelling and pain three weeks after 2019-nCoV infection. The swelling extended from the upper clavicle to the upper abdomen. Delayed inflammation reaction was diagnosed, followed by emergency surgery. During the surgery, the injected material was observed to gush out in a form of greenish and thin paste, with a foul odor. The wound had extensive diffuse oozing and hemoglobin decreased significantly. The patient was discharged one week after surgery. During a three-month follow-up, she experienced weakness and fatigue with no complications. There are still many unknown areas of the impact of 2019-nCoV on human foreign bodies and immune system, which is diverse and complex. Reporting various possible symptoms and mechanisms is conducive to improving the level of awareness and intervention of plastic surgeons.

    查看更多>>摘要:该文报道了1例应用双侧改良水平推进皮瓣修复眉间鼻根恶性肿瘤切除后组织缺损病例。患者女,20岁,因"眉间鼻根肿物5年余,增大、破溃5个月"入院。术中首先行眉间鼻根皮肤肿物切除,术中快速冰冻病理结果回报为基底细胞癌,切缘未累及,遂行双侧改良水平推进皮瓣修复术。术后3个月患者术区愈合良好,外观形态、眼睑功能正常,基底细胞癌无复发迹象。应用双侧改良水平推进皮瓣修复眉间鼻根部位基底细胞癌切除后的组织缺损效果良好。 This paper reports a case of reparing the tissue defect after resection of a malignant tumor in the glabellar nasion with bilateral modified horizontal advancement flaps. The patient, a 20-year-old female, was admitted to the hospital because of a mass on the nasion between the eyebrows for more than 5 years, with enlargement and ulceration for 5 months. Surgical resection of the tumor was performed. Intraoperative rapid frozen sections showed that the tumor was basal cell carcinoma, and there were no cancer cells at the margins. Bilateral modified horizontal advancement flap repair was performed. At three months after the operation, the incision area healed well, the appearance and eyelid function were normal, and there was no sign of recurrence of basal cell carcinoma. Bilateral modified horizontal advancement flap was effective in repairing the tissue defect after resection of glabellar nasal root basal cell carcinoma.

    查看更多>>摘要:面颈部除皱手术是面部年轻化各种治疗手段中的一种。然而,就其涉及的面部解剖结构之多,临床操作过程之复杂多样,以及术后不可预知的风险之大而言,是一项极具挑战性的外科手术。西方发达国家的面部除皱手术始于20世纪初,手术旨在通过外科手段改善面颈部皮肤、筋膜、脂肪等组织所形成的衰老外观。我国面颈部除皱手术起步较晚,是由中国医学科学院整形外科医院宋业光教授等医学前辈于20世纪80年代末至90年代初率先拓荒。之后,一大批创业者承前启后,对我国面部除皱手术的系统化、规范化做出了重要贡献。在对面部老化机制、面颈部解剖和组织结构变化深入研究的基础上,我国的面部除皱手术经历了一个由简到繁、日渐完善的发展过程。再后来,经过对除皱手术长期效果的观察和手术方案的不断完善,又经历了一个由繁到减、日臻成熟的过程。该文通过回顾历年的文献,对我国面颈部除皱手术的发展历程进行了阶段性的概括和梳理。 Facial rhytidectomy is one of the most complex and unpredictable procedures in the field of cosmetic and plastic surgery, aiming to rejuvenate the face. The facial rejuvenation surgery began in the early 20th century. The purpose of the surgery is to alleviate the aging characteristics of the skin, fascia, fat and other tissues of the face and neck by surgical means. In China, due to historical development and other factors, it started late. From the late 1980s to the early 1990s, Song Yeguang and other pioneers took the lead in venturing into this field. With the significant growth of the economy and the increasing attention from both medical professionals and marketing to cosmetic surgery, surgeons in this era have made substantial contributions to the systematic and standardized advancement of rhytidectomy surgery. However, with the understanding of the aging process of the face and the in-depth study of the changes in the anatomy and tissue structure of the face and neck, surgical techniques have evolved from simple to complex, and gradually improving over time. Through long-term observation of the surgical outcomes, the surgical technique has evolved from complex to simple. The purpose of this paper is to summarize the evolution of rhytidectomy in China by reviewing the literature from previous years.

    查看更多>>摘要:脂肪源性间充质干细胞(ADSC)因其存量丰富、易于获取、免疫原性低等特点,已经成为近年来再生医学和干细胞领域的研究热点。ADSC不仅能与T细胞、巨噬细胞和B细胞等免疫细胞作用而直接参与调节免疫系统,还能通过分泌可溶性细胞因子,如白介素、生长因子和细胞外囊泡等进行间接调节。ADSC的免疫调节作用已经在多项体内、外实验中得到证实,也已被尝试应用于临床治疗多种免疫相关性疾病并取得良好效果,如硬皮病、系统性红斑狼疮和特应性皮炎等。该文就ADSC免疫调节作用的直接机制、间接机制以及临床应用进行总结,并对其研究方向与应用前景进行展望。 Adipose-derived mesenchymal stem cell (ADSC) has become a research hotspot in the field of regenerative medicine and stem cells in recent years because of its rich stock, easy access and low immunogenicity. ADSC can regulate immune system not only directly by interacting with immune cells, such as T cells, macrophages and B cells, but also indirectly via secreting soluble cytokines, such as interleukin (IL), growth factors and extracellular vesicles. Meanwhile, the immunomodulatory effects of ADSC have been demonstrated in vitro and in vivo experiments and applied to treat various of immune-related diseases with good results, such as systemic scleroderma, systemic lupus erythematosus and atopic dermatitis, etc. In this article, the direct and indirect immunomodulatory mechanisms and clinical applications of ADSC are described, its research direction and application prospect are also prospected.

    查看更多>>摘要:乳腺肿瘤整形手术包括整形保乳术、乳房切除后再造术及局部晚期乳腺癌切除后胸壁重建手术,自体皮瓣在乳房切除后再造术中的应用已被广泛讨论,但在另外2种手术中的应用情况却鲜有总结。该文从临床解剖及应用、并发症、美学效果、肿瘤学安全性等方面,对相关皮瓣的应用进展进行综述,以期为临床医生选择术式提供依据。 Oncoplastic breast surgery includes oncoplastic breast-conserving surgery, breast reconstruction surgery after mastectomy, and chest wall reconstrcution after resection of locally advanced breast cancer. The application of autologous flaps in breast reconstruction surgery has been widely acknowledged, but little was known about their applications in the other two procedures. This paper reviewed the application progress of the relevant flaps in terms of clinical anatomy, application, complication, aesthetic outcome, and oncological safety, to provide evidence for clinicians’ decisions.

    查看更多>>摘要:长链非编码RNA(lncRNA)是一种主要分布在多种真核生物细胞中的长度大于200个核苷酸的非编码RNA,其本身不能编码蛋白质。静脉畸形作为一类先天性脉管畸形,随着基因测序技术的发展,对其分子发病机制了解日渐深入。lncRNA的异常表达可能与静脉畸形的发生、发展有密切联系。lncRNA可选择性调控血管内皮细胞和血管平滑肌细胞的增殖、凋亡及迁移等,从而影响静脉畸形的发生、发展。lncRNA有望成为静脉畸形一类新的诊断标志物或新的治疗靶点。 Long non-coding RNA(lncRNA) is a kind of non-coding RNA with a length of more than 200 nucleotides mainly distributed in a variety of eukaryotic cells. It cannot encode proteins itself. With the development of gene sequencing technology, the molecular pathogenesis of venous malformation is getting deeper and deeper. Abnormal expression of lncRNA may be closely related to the occurrence and development of venous malformations. LncRNA can selectively regulate the proliferation, apoptosis and migration of endothelial cells and vascular smooth muscle cells, thus the occurrence and development of venous malformations is affected. Therefore, lncRNA is expected to be a novel diagnostic marker or a new therapeutic target for venous malformations.

    查看更多>>摘要:创面愈合瘢痕形成对患者的正常生活及心理健康造成影响,是整形外科中的棘手问题。近年来机械力及其信号传导对创口愈合的影响机制受到广泛关注,并为防治病理性瘢痕的相关研究提供了新思路。该文综述了机械牵张力对不同时期创面愈合影响机制的研究进展,在伤口愈合的止血炎症期、增殖期和组织重塑期3个阶段中,机械牵张力通过复杂的信号传导与反馈机制引起慢性炎症、纤维化增生、血管形成、细胞外基质重塑等生物学反应,对伤口愈合和瘢痕形成造成影响。该文还综述了近年临床与实验室关于创面愈合瘢痕形成的机械牵张力治疗研究的进展。 The formation of pathological scar has an impact on patients’ normal life and mental health, which is also a thorny problem in plastic surgery. In recent years, the influence mechanism of mechanical tension and signal transduction on wound healing has attracted extensive attention and provides a new idea for the prevention and treatment of pathological scar. This paper reviews the research progress of the influence mechanism of mechanical stretch on wound healing in different stages. In the inflammatory stage, hyperplasia stage and tissue remodeling stage of wound healing, mechanical stretch tension causes biological reactions such as chronic inflammation, fibrosis, angiogenesis, and extracellular matrix remodeling through complex signal transduction and feedback mechanisms, which has an impact on wound healing and scar formation. Mechanical stretch therapy for wound healing and scar formation has also achieved certain result in clinical and basic study.
