    查看更多>>摘要:OBJECTIVE:To explore the material basis of the difference of efficacy of Dahuang(Radix Et Rhizoma Rhei Palmati)-Taoren(Semen Persicae)(DT)drugs with different proportions.METHODS:Samples of different ratios of Dahuang(Radix et Rhizoma Rhei Palnati,DH)to Taoren(Semen Persicae,TR)(Group A 1∶1,B 2∶3,C 3∶2)were analyzed based on gas chromatography time-of-flight mass spectrometry untargeted metabolomics technique.RESULTS:A total of 240 primary metabolites were detected.Forty-one differential metabolites involved nine differential metabolic pathways,of which four were closely related to the efficacy of DT in the treatment of heat and blood stasis syndrome.These pathways included the biosynthesis of amino acid(phenylalanine tyrosine and tryptophan),flavonoids,unsaturated fatty acids,and the glycolysis/glycogenesis pathway.CONCLUSION:There are significant differences in the efficacy of different ratios of DT drugs,and their optimal ratio for the treatment of heat and blood stasis syndrome should be 1∶1.

    Individualized Traditional Chinese Medicine treatment vs antibiotics for recurrent urinary tract infections:a multicenter,randomized controlled study

    查看更多>>摘要:OBJECTIVE:To systematically assess the effects of individualized Chinese medicines on recurrent urinary tract infections(rUTIs).METHODS:This study recruited 230 adult female patients in the remission phase of rUTIs from five hospitals in China.The patients were randomly allocated to two groups:an individualized Chinese medicine group(n=114)and a control group(n=116).Patients in the Chinese medicine group received individualized Chinese herbs,which were evaluated for syndrome differentiation.Patients in the control group received antibiotic treatment combined with a Chinese medicine placebo.The duration of treatment was three courses of four weeks each,with a three-month subsequent follow-up.UTI recurrence rate,Traditional Chinese Medicine(TCM)syndrome scores,36-item Short Form Survey(SF-36)score,and urine secretory immunoglobulin A(SlgA)were measured and analyzed before and after treatment in each group.RESULTS:Patients from the Chinese medicine group exhibited significant decreases in both short-and long-term UTI recurrence rates compared with the control group(P<0.05).The changes in TCM syndrome scores between the Chinese medicine and control groups were significant(P<0.05).The changes in the average SF-36 quality-of-life scores in the Chinese medicine group were also significantly higher than those in the control group after treatment(P<0.05).The Chinese medicine group also demonstrated a significant increase in urine SlgA expression.CONCLUSION:Taken together,compared to the often-used long-term antimicrobial prophylaxis during the remission stage of rUTIs,treating patients with an individualized Chinese medicine decoction by syndrome differentiation could effectively reduce the recurrence rate,improve the patients'TCM syndrome scores and quality of life,and enhance immunity,which in turn helps to prevent antibiotic resistance.

    Efficacy of Daoyin combined with lower limb robot as a comprehensive rehabilitation intervention for stroke patients:a randomized controlled trial

    查看更多>>摘要:OBJECTIVE:To assess the effectiveness of a comprehensive rehabilitation approach combining Traditional Chinese Medicine Daoyin with lower limb robotics during the recovery phase of stroke patients.METHODS:Stroke patients meeting the specified criteria were randomly assigned to one of four groups using a random number table:Control group,Daoyin group,lower limb robot group(LLR group),and Daoyin and lower limb robot group(DLLR group).Each group received distinct treatments based on conventional rehabilitation training.The treatment duration spanned two weeks with two days of rest per week.Pre-and post-intervention assessments included various scales:Fugl-Meyer Assessment(FMA),Berg balance scale(BBS),Barthel index(Bl),Fatigue Scale-14(FS-14),Pittsburgh sleep quality index(PSQI),Hamilton Anxiety Scale(HAMA),and Hamilton Depression Scale(HAMD).RESULTS:Statistically significant differences were observed in the lower limb function measured by FAM between the Control group(15±5)and the DLLR group(18±5)(P=0.049).In the Barthel index,a statistically significant difference was noted between the Control group(54±18)and the DLLR group(64±11)(P=0.041).Additionally,significant differences were found in the Berg balance scale between the Control group(21±10)and the DLLR group(27±8)(P=0.024),as well as between the Control group(21±10)and the LLR group(26±10)(P=0.048).CONCLUSION:The findings of this study suggest that the combined use of Daoyin and robotics not only enhances motor function in stroke patients but also has a positive impact on fatigue,sleep quality,and mood.This approach may offer a more effective rehabilitation strategy for stroke patients.

    查看更多>>摘要:OBJECTIVE:To explore the early hemostatic mechanism of Jianpi Yiqi Shexue decoction(健脾益气摄血方,JYSD)in treating immune thrombocytopathy(ITP),based on the functional homeostasis of brain-intestine axis and blood neurotransmitterMETHODS:Non-drug treatment cases:Healthy volunteers were selected as normal control group and compared with patients with dysfunctional uterine bleeding,gastrointestinal tumors with bleeding and ITP,to detect the changes of blood 5-hydroxytryptamine(5-HT),β-endorphin(β-EP),vasoactive intestinal peptide(VIP)and compare the changes of blood neuro-transmitters in patients with different disease symptoms.Drug treatment cases:According to the randomized controlled multicenter clinical trial,272 ITP patients were randomly divided into three groups:treatment group(JYSD)combined group(JYSD+Prednisone)control group(Prednisone).The changes of blood neuro-transmitter(5-HT,β-EP,VIP)before and after treatment were detected on the basis of peripheral blood platelet(PLT)and grade score.RESULTS:Non-drug treatment cases:compared with the normal control group,the 5-HT level was higher,and the VIP and β-EP levels were both lower in the ITP group(P<0.001),and the 5-HT,VIP and β-EP levels in the Gastrointestinal tumors with bleeding group were also lower compared with the normal control group(P<0.05,0.001).Drug treatment cases:The PLT grading scores of the combination group and the control group after treatment were lower than that before treatment(P<0.05,0.001).The PLT grading score of the 3 groups were compared in pairs after treatment:the combination group was the lowest among the 3 groups,which was better than the treatment group,but no better than the control group(vs the treatment group,P=0.005,vs the control group,P=0.709).The statistical results of full analysis set(FAS)and per protocol set(PPS)were consistent.The bleeding symptom scores of the treatment and combination groups began to drop 7 d after treatment,and kept dropping 14 d after treatment until the end of the study(P<0.05).On the other hand,the control group started to show favorable results 14 d after treatment(P<0.05).The FAS and PPS analysis results were consistent.In the control group,the 5-HT level was higher and VIP level was lower after treatment,compared with those before treatment(P<0.05,0.001).The β-EP levels were both increased in the treatment and combination group after treatment,compared with those before treatment(P<0.05).After treatment,the β-EP levels in the treatment and control groups were significantly lower compared with the combination groups(P<0.05).After treatment,compared with the control group,the VIP levels in the treatment and combination groups were up-regulated,and the differences were statistically significant by rank sum test(P<0.01),and by t-test(P=0.0002,0.0001).CONCLUSIONS:The prednisone tablet is better than the JYSD in increasing the level of PLT,while prednisone tablet combined with JYSD has more advantages in improving patients'peripheral blood PLT levels.However,in improving the bleeding time of ITP patients,the combination of the two drugs was significantly delayed compared with the single usage,showing the characteristics and advantages of traditional Chinese medicine.JYSD can regulate the neurotransmitter level of ITP patients through the function of the brain-gut axis,mobilize 5-HT in the blood of ITP patients to promote the contraction of blood vessels and smooth muscles,and activate the coagulation mechanism are the early hemostatic mechanisms of JYSD.Up-regulate the levels of β-EP and balancing VIP levels may be an important part of the immune mechanism of JYSD for regulating ITP patients.

    查看更多>>摘要:OBJECTIVE:To evaluate the clinical efficacy and safety of Shenzhu Guanxin recipe granules(参术冠心颗粒,SGR)in treating patients with intermediate coronary lesions(ICL),and to investigate the potential mechanism though a transcriptome sequencing approach.METHODS:ICL patients with Qi deficiency and phlegm stasis were adopted and randomly assigned to a case group or a control by random number generator in a 1:1 randomization ratio to evaluate the clinical efficacy.RESULTS:There was no significant difference between the two groups in coronary computed tomography angiography related indexes in the two groups before and after intervention.Through the gene chip expression analysis,it is finally concluded that there are 355 differential mRNAs(190 up-regulated genes and 165 down regulated genes)when compared the SGR group and placebo group.Through protein-protein interaction network analysis of differentially expressed genes,10 hub genes were finally obtained:CACNA2D2,CACNA2D3,DNAJC6,FGF12,SGSM2,CACNA1G,LRP6,KIF25,OXTR,UPB1.CONCLUSIONS:SGR combined with Western Medicine can be safely used to treat ICL patients with Qi deficiency and phlegm stasis.The possible mechanism of action and relevant gene loci and pathway were proposed.

    查看更多>>摘要:OBJECTIVE:To investigate the underlying protein molecular mechanisms of"Qi stagnation and blood stasis syndrome"(QS)and"Qi deficiency and blood stasis syndrome"(QD),as two subtypes of coronary artery disease(CAD)in Traditional Chinese Medicine(TCM),following percutaneous coronary intervention(PCI).METHODS:In this study,a total of 227 CAD patients with QS and 211 CAD patients with QD were enrolled;all participants underwent PCI.Label-free quantification proteomics were employed to analyze the changes in serum in two subtypes of CAD patients before and 6 months after PCI,aiming to elucidate the intervention mechanism of PCI in treating CAD characterized by two different TCM syndromes.RESULTS:Biochemical analysis revealed significant changes in tumor necrosis factor-α,high density lipoprotein cholesterol,blood stasis clinical symptoms observation,and Gensini levels in both patient groups post-PCI;Proteomic analysis identified 79 and 95 differentially expressed proteins in the QS and QD patient groups,respectively,compared to their control groups.complement C8 alpha chain,complement factor H,apolipoprotein H,apolipoprotein B,plasminogen,carbonic anhydrase 2,and complement factor I were altered in both comparison groups.Furthermore,enrichment analysis demonstrated that cell adhesion and connectivity-related processes underwent changes in QS patients post-PCI,whereas lipid metabolism-related pathways,including the peroxisome proliferator-activated receptor signaling pathway and extracellular matrix receptor interaction,underwent changes in the QD group.The protein-protein interaction network analysis further enriched 52 node proteins,including apolipoprotein B,lipoprotein(a),complement C5,apolipoprotein A4,complement C8 alpha chain,complement C8 beta chain,complement C8 gamma chain,apolipoprotein H,apolipoprotein A-Ⅱ,albumin,complement C4-B,apolipoprotein C3,among others.The functional network of these proteins is posited to contribute to the pathophysiology of CAD characterized by TCM syndromes.CONCLUSION:The current quantitative proteomic study has preliminarily identified biomarkers of CAD in different TCM subtypes treated with PCI,potentially laying the groundwork for understanding the protein profiles associated with the treatment of various TCM subtypes of CAD.

    Study on blood pressure rhythm in hypertensive patients with Yin deficiency syndrome and a random forest model for predicting hypertension with Yin deficiency syndrome

    查看更多>>摘要:OBJECTIVE:To investigate blood pressure rhythm(BPR)in Yin deficiency syndrome of hypertension(YDSH)patients and develop a random forest model for predicting YDSH.METHODS:Our study was consistent with technical processes and specification for developing guidelines of Evidence-based Chinese medicine clinical practice(T/CACM 1032-2017).We enrolled 234 patients who had been diagnosed with primary hypertension without antihypertensive medications prior to the enrollment.All participants were divided into Yin deficiency group(YX,n=74)and non-Yin deficiency group(NYX,n=160).Participants were professionally grouped by three experienced chief Traditional Chinese Medicine(TCM)physicians according to four examinations(i.e.,inspection,listening and smelling,inquiry and palpation).We collected data on 24 h ambulatory blood pressure monitoring(ABPM)and YDSH rating scale.We divided 24 h of a day into 12 two-hour periods[Chen-Shi(7:00-9:00),Si-Shi(9:00-11:00),Wu-Shi(11:00-13:00),Wei-Shi(13:00-15:00),Shen-Shi(15:00-17:00),You-Shi(17:00-19:00),Xu-Shi(19:00-21:00),Hai-Shi(21:00-23:00),Zi-Shi(23:00-1:00),Chou-Shi(1:00-3:00),Yin-Shi(3:00-5:00),Mao-Shi(5:00-7:00)]according to the theory of"midnight-midday ebb flow".We used random forest to build the diagnostic model of YDSH,with whether it was Yin deficiency syndrome as the outcome.RESULTS:Compared with NYX group,YX group had more female participants with older age,lower waist circumference,body mass index(BMI),diastolic blood pressure(DBP),and smoking and drinking rate(all P<0.05).The YDSH rating scores of YX group[28.5(21.0-36.0)]were significantly higher than NYX group[13.0(8.0-22.0)](P<0.001),and the typical symptoms of YX group included vexing heat in the chest,palms and soles,dizziness,dry eyes,string-like and fine pulse,soreness and weakness of lumbus and knees,palpitations,reddened cheeks,and tinnitus(all P<0.05).The ratio of non-dipper hypertension in YX group was higher than in NYX group(56.9%vs 44.4%,P=0.004).Compared with NYX group,24 h DBP standard deviation(SD),nighttime DBP SD,Si-Shi DBP,Si-Shi mean arterial pressure(MAP),Hi-Shi systolic blood pressure(SBP),Hi-Shi DBP,Hi-Shi MAP,Zi-Shi SBP,Zi-Shi DBP,Zi-Shi MAP,Chou-Shi SBP SD,Chou-Shi DBP SD,Chou-Shi SBP coefficient of variation(CV)were lower in YX group(all P<0.05).Binary Logistic Regression analysis showed that the diagnosis of YDSH was positively correlated with age,heart rate,YDSH rating scores,and four TCM symptoms including vexing heat in the chest,palms and soles,string-like and fine pulse,soreness and weakness of lumbus and knees,and reddened cheeks(all P<0.05),but was negatively correlated with smoking(P>0.05).In addition,the diagnosis of YDSH was positively correlated with daytime SBP SD,nighttime SBP SD,nighttime SBP CV,and Hi-Shi SBP CV,but was negatively correlated with 24 h SBP CV,daytime DBP SD,nighttime DBP SD,and Hi-Shi DBP(all P<0.05).Hi-Shi SBP CV had independent and positive correlation with the diagnosis of YDSH after adjusting the variables of age,gender,course of hypertension,BMI,waist circumference,SBP,DBP,heart rate,smoking and drinking(P=0.029).Diagnostic model of YDSH was established and verified based on the random forest.The results showed that the calculation accuracy,specificity and sensitivity were 77.3%,77.8%and 76.9%,respectively.CONCLUSION:The BPR was significantly attenuated in YDSH patients,including lower 24 h DBP SD and nighttime DBP SD,and Hi-Shi SBP CV is independently correlated with the diagnosis of YDSH.The prediction accuracy of diagnosis model of YDSH based on the random forest was good,which could be valuable for clinicians to differentiate YDSH and non-Yin deficiency patients for more effective hypertensive treatment of TCM.

    Distribution of Traditional Chinese Medicine syndromes in diabetic kidney disease chronic kidney disease 1-5:a correlation study

    查看更多>>摘要:OBJECTIVE:To analyze the distribution of Traditional Chinese medicine(TCM)syndromes in patients with diabetic kidney disease(DKD)and its related factors.METHODS:We enrolled 435 patients with DKD,who were not undergoing dialysis,admitted to the Department of Nephrology,First Medical Center,Chinese PLA General Hospital from April 2020 to August 2021.Analysis of their TCM syndromes and related factors was carried out.RESULTS:The 435 patients included 109,117,86,and 123 chronic kidney disease(CKD)1-2,CKD3,CKD4,and CKD5 cases,respectively.With the progression of CKD1-5,the proportion of Yin deficiency and dry heat syndrome,and that of Qi and Yin deficiency syndrome showed a downward trend,whereas the proportion of spleen-kidney Yang deficiency,blood deficiency,blood stasis,water stagnation,and phlegm turbidity syndromes showed an upward trend;the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that Yin deficiency and dry heat syndrome was positively correlated with hemoglobin[odds ratio(OR)=1.022,P=0.005],albumin(OR=1.058,P=0.006),and estimated glomerular filtration rate(eGFR)(OR=1.020,P<0.001)but negatively correlated with male sex(OR=0.277,P=0.004).Qi and Yin deficiency syndrome was positively correlated with albumin(OR=1.056,P<0.001)and eGFR(OR=1.008,P=0.022)but negatively correlated with age(OR=0.977,P=0.023).Liver-kidney Yin deficiency syndrome was positively correlated with age(OR=1.028,P=0.021)and glycosylated hemoglobin(OR=1.223,P=0.007)but negatively correlated with total cholesterol(OR=0.792,P=0.006).Spleen-kidney Yang deficiency syndrome was negatively correlated with hemoglobin(OR=0.977,P<0.001),albumin(OR=0.891,P<0.001),and eGFR(OR=0.978,P<0.001)but positively correlated with high density lipoprotein(OR=3.376,P=0.001).CONCLUSION:With CKD1-5 progression,TCM syndromes changed from Yin deficiency and dry heat syndrome to syndrome of deficiency of both Qi and Yin,liver-kidney Yin,and spleen-kidney Yang deficiency syndromes.TCM syndromes were correlated with laboratory test results.

    查看更多>>摘要:OBJECTIVE:To assess the efficacy and safety of Neiguan(PC6)acupoint acustimulation in preventing chemotherapy-induced nausea and vomiting(CINV),especially for patients with guideline-inconsistent CINV prophylaxis(GICP)due to personal reasonsMETHODS:From January 2021 to December 2021,373 patients suffered from solid malignancy were recruited according to the inclusion criteria.Complete response(no emesis and no rescue medication use)rate during the overall phase(0-120 h of each chemo-cycle)was the primary assessment of CINV control.The Functional Living Index-Emesis(FLIE)questionnaire was investigated among these patients as a secondary'quality of life'objective to assess the impact of CINV on patients'daily life by recording score of nausea and vomiting.RESULTS:With acustimulation of Neiguan(PC6)acupuncture point through a portable,noninvasive and user-friendly device,in terms of complete response rate and scores in nausea/vomiting by FLIE questionnaire,patients achieve a better outcome in highly emetogenic chemotherapy(HEC)induced CINV,especially GICP subgroup.Meanwhile,analysis also demonstrated this tendency existed in other patients with HEC/GCCP(guideline consistent CINV prophylaxis)and moderate emetogenic chemotherapy,although the difference was not significant.CONCLUSION:Considering advantages of Neiguan(PC6)acustimulation such as noninvasive,covered by medical insurance and few side effects,we believe it would be an ideal auxiliary tool in CINV control,especially in patients who receive highly emetogenic chemo-protocol and are reluctant to GCCP for economic reasons.

    Effect of pediatric Tuina on children's recurrent acute respiratory tract infections:a retrospective cohort study in Southern China

    查看更多>>摘要:OBJECTIVE:To determine the effectiveness of pediatric Tuina(PT)in preventing recurrent acute respiratory tract infections(ARTIs)in children.METHODS:This is a retrospective cohort study based on the electronic medical records of children with recurrent ARTIs in 2016.Children were divided into a PT group or a non-PT group,according to whether they had received PT or not in 2016.The primary outcome was the number of ARTI episodes in 2017 and 2018.The secondary outcomes were the number of ARTIs leading to outpatient department visits and outpatient antibiotic prescriptions due to ARTIs in the same time period.Negative binomial regressions were used to detect the association between PT and the outcomes.RESULTS:A total of 2303 children were included in the analysis,including 94 in the PT group and 2209 in the non-PT group.Children who received PT six or more times in 2016 had fewer episodes of ARTIs in 2017[incidence rate ratio(IRR):0.59,95%confidence interval(CI)(0.42-0.84)]and 2018[IRR:0.58,95%CI(0.36-0.94)]and fewer outpatient department visits due to ARTIs in 2017[IRR:0.56,95%CI(0.38-0.83)]than children who had not received PT in 2016.There was no significant difference in the number of outpatient antibiotic prescriptions between the two groups.CONCLUSIONS:Receiving PT six or more times within one year is associated with a decrease in recurrent ARTIs in children in the following two years.Randomized controlled trials are needed for effect evaluation prior to establishing PT as a method for preventing recurrent ARTIs among children.
