    查看更多>>摘要:目的 观察钠-葡萄糖协同转运蛋白2抑制剂(SGLT-2i)卡格列净对超重及肥胖的2型糖尿病(T2DM)患者骨转换标志物的影响。 方法 选取2019年10月9日至2020年12月28日住院的42例超重及肥胖T2DM患者,随机分为实验组(n=18)和对照组(n=24),实验组应用卡格列净联合其他类降糖药物,对照组应用非SGLT-2i类降糖药物,比较两组治疗前及治疗24周后血清Ⅰ型胶原N端前肽(PINP)及Ⅰ型胶原C末端肽交联(β-CTX)的变化。 结果 治疗后两组比较,PINP无显著差异,实验组β-CTX水平低于对照组(P<0.05)。 结论 应用卡格列净对超重及肥胖T2DM患者骨形成无明显影响,可能对骨吸收有一定抑制作用。 Objective To observe the effects of sodium-glucose cotransporter 2 inhibitor (SGLT-2i) Canagliflozin on bone turnover markers in overweight and obese patients with type 2 diabetes (T2DM). Methods A total of 42 overweight and obese T2DM patients (BMI≥25 kg/m2) who were hospitalized in our hospital from October 9, 2019, to December 28, 2020, were selected and randomly divided into experimental group (n=18) and control group (n=24). The experimental group was treated with Canagliflozin, while the control group was treated with non-SGLT-2 i. The changes of serum PINP and β-CTX levels were compared between the two groups before and after treatment for 24 weeks. Results After treatment, there was no significant difference in PINP between the two groups, while β-CTX was lower in the experimental group than that in the control group. Conclusions Canagliflozin has no significant effect on bone formation but may inhibit bone resorption in overweight and obese T2DM patients.

    查看更多>>摘要:目的 探讨腰椎磁共振成像(MRI)椎体骨质量(VBQ)评分评估糖尿病绝经后女性骨密度的应用价值。 方法 回顾性分析2019年6月至2022年12月在天津医科大学朱宪彝纪念医院诊治的糖尿病绝经后女性145例,根据骨密度结果分为正常组、骨量减少组和骨质疏松组。通过腰椎MRI T1加权像测量VBQ评分,Image J软件计算L3椎体右侧椎旁肌脂肪浸润分数。比较3组各参数的差异,受试者工作特征(ROC)曲线分析VBQ评分诊断骨质疏松的效能,分析VBQ评分和T值及各参数的相关性。 结果 VBQ评分骨质疏松组>骨量减少组>正常组(P均<0.05)。L3椎体右侧椎旁肌脂肪浸润分数骨质疏松组大于骨量减少组、正常组(P均<0.05)。VBQ评分诊断骨质疏松的ROC曲线下面积为0.830。VBQ评分与L3椎体右侧椎旁肌脂肪浸润分数呈显著弱正相关。VBQ评分与双能X线吸收仪(DXA)最低T值呈显著中度负相关。 结论 VBQ评分对糖尿病绝经后女性骨密度的评估可能有一定的价值。 Objective To explore the value of vertebral bone quality (VBQ) score of lumbar spine MRI in evaluating bone density of postmenopausal women with diabetes. Methods This was a retrospective analysis of 145 postmenopausal women with diabetes admitted to our hospital from June 2019 to December 2022. According to the results of bone density, they were divided into normal bone, osteopenia, and osteoporosis groups. The VBQ score was measured by T1-weighted MRI of the lumbar spine. The fat infiltration fraction of the right paraspinal muscle at L3 was calculated through Image J software. The differences of parameters among the three groups were compared. The receiver operating characteristic (ROC) curve was used to evaluate the efficiency of VBQ score in the diagnosis of osteoporosis. The correlation between VBQ score and T value and various parameters were analyzed. Results The VBQ score of osteoporosis group was higher than osteopenia group and normal group (all P<0.05). The fat infiltration fraction of L3 right paraspinal muscle in osteoporosis group was higher than osteopenia group and normal group (allP<0.05). The area under the ROC curve for diagnosing osteoporosis by VBQ score was 0.830. The VBQ score was significantly positively correlated with the fat infiltration fraction of the right paraspinal muscle at L3. The VBQ score was significantly and moderately negatively correlated with the lowest T value of DXA. Conclusion The VBQ score may be valuable in evaluating bone density of postmenopausal women with diabetes.

    查看更多>>摘要:目的 探讨细针穿刺(FNA)洗脱液甲状旁腺激素(PTH)测定(FNA-PTH)对原发性甲状旁腺功能亢进病灶(HPTL)的诊断价值。 方法 选择2018年10月至2023年2月经颈部超声检出可疑甲状旁腺病变的患者,结合血PTH、核素显像、术后病理结果判定HPTL,分析FNA-PTH对HPTL的诊断效能,并构建合理的诊断切点。 结果 本研究共纳入94例患者、98个结节,其中76例患者(80个结节)接受了FNA检查。HPTL与非HPTL的FNA-PTH中位数分别为2 396.50 pg/ml(Q1~Q3:878.08~5 000.00 pg/ml)、21.98 pg/ml(16.04~32.16 pg/ml),FNA-PTH/血清PTH比值(PTH比值)中位数分别为12.47(4.89~29.43)、0.62(0.45~0.79),差异均有统计学意义(P值均<0.001)。ROC曲线显示FNA-PTH测值的AUC略高于PTH比值(0.937比0.849)。以187.90 pg/ml作为FNA-PTH的诊断界值时敏感性为88.9%,特异性与准确性均为90.9%,与PTH比值无统计学差异(P=1.0)。 结论 FNA-PTH检测对HPTL具有很高的定位与诊断价值,以187.90 pg/ml作为诊断切点效能最佳。 Objective To investigate the diagnostic value of parathyroid hormone measurement in fine needle aspiration washout (FNA-PTH) in primary hyperparathyroidism lesions (HPTLs). Methods Patients with suspected parathyroid lesions detected by neck ultrasonography from October 2018 to February 2023 were selected, and HPTL was determined based on serum parathyroid hormone (PTH), radionuclide imaging, and postoperative pathological results. The diagnostic value of FNA-PTH on HPTL was analyzed, and an effective diagnostic cut-off point was also established. Results A total of 94 patients and 98 nodules were enrolled in this study, of which 76 patients (80 nodules) underwent FNA. The median values of FNA-PTH for HPTLs and non-HPTLs were 2 396.50 pg/ml (878.08-5 000.00 pg/ml), 21.98 pg/ml (16.04-32.16 pg/ml), and FNA-PTH/serum PTH (PTH ratio) were 12.47 (4.89-29.43) and 0.62 (0.45-0.79), respectively the differences were statistically significant (all P<0.001). The ROC curve showed that the AUC of the FNA-PTH value was slightly higher than the PTH ratio (0.937 vs 0.849). Taking 187.90 pg/ml as the diagnostic cut-off value, the sensitivity, specificity, and accuracy of FNA-PTH were 88.9%, 90.9%, and 90.0%, respectively. The efficacy was slightly better than the PTH ratio without statistical difference. Conclusions FNA-PTH testing is of great value in the localization and diagnosis of HPTL, with 187.90pg/ml as the optimal diagnostic cut-off.

    查看更多>>摘要:序列相似家族19成员A5(family with sequence similarity 19, member A5,FAM19A5)是近年来新发现的脂肪因子。研究发现,FAM19A5与肥胖、胰岛素抵抗、非酒精性脂肪肝病等代谢性疾病密切相关,这表明FAM19A5有希望成为新型代谢性疾病生物标记物。本文对FAM19A5与代谢性疾病的相关性及可能的作用机制进行归纳综述,以期为FAM19A5的相关研究提供线索。 Family with sequence similarity 19, member A5 (FAM19A5) is a newly discovered adipokine in recent years. It has been found that FAM19A5 is closely associated with metabolic diseases such as obesity, insulin resistance, and non-alcoholic fatty liver disease, which suggests that FAM19A5 is promising as a novel metabolic disease biomarker. In this paper, we summarize and review the correlation between FAM19A5 and metabolic diseases and the possible mechanisms of action, in order to provide clues for the related research of FAM19A5.

    查看更多>>摘要:3β-羟基类固醇脱氢酶(3β-hydroxysteroid dehydrogenase,3β-HSD)参与多种甾体激素的合成,是影响代谢性疾病发生发展的重要生物标志物,以3β-HSD作为治疗靶点的药物已经成为了近年的研究热点。此文就3β-HSD在糖尿病、肥胖、高血压、多囊卵巢综合征以及非酒精性脂肪肝中潜在生理病理机制及临床应用前景的最新研究进展进行总结。 3β-hydroxysteroid dehydrogenase (3β-HSD) participates in the synthesis of a variety of steroid hormones and acts as an important biomarker that affects the occurrence and development of metabolic diseases. Drugs targeting 3β-HSD have become a research hotspot in recent years. In this paper, we summarize the latest research of the potential pathophysiological mechanism and clinical application prospect of 3β-HSD in diabetes, obesity, hypertension, polycystic ovary syndrome and nonalcoholic fatty liver disease.

    查看更多>>摘要:MOTS-c是线粒体DNA(mtDNA)短开放阅读框(sORF)编码的小分子多肽,由线粒体12S rRNA编码,作为最新被发现的线粒体衍生肽(MDPs),是目前研究的热点,其可以改善胰岛素抵抗(IR)预防2型糖尿病(T2DM);增加棕色脂肪(BAT)产热,促进白色脂肪(WAT)棕色化,以适应寒冷;防止肥胖和脂质代谢紊乱;降低非酒精性脂肪性肝病(NAFLD)的发生率;保护血管内皮细胞,预防冠心病,延缓骨质疏松(OP)。MOTS-c对改善多种代谢相关性疾病的不良结局发挥着积极作用,本文主要围绕MOTS-c与代谢相关性疾病的关系及其作用机理展开综述。 MOTS-c is a small peptide encoded by the short open reading frame(sORF) within mitochondrial DNA(mtDNA), which is encoded by the mitochondrial 12S rRNA. As one of the newly discovered mitochondrial-derived peptides(MDPs), it has emerged as a focal point in research endeavors. MOTS-c has been shown to improve insulin resistance(IR) and prevent type 2 diabetes mellitus(T2DM). Moreover, MOTS-c elicits an augmentation in brown adipose tissue(BAT) thermogenesis and fosters the browning of white adipose tissue(WAT), thereby enabling adaptation to cold environments. It can also prevent obesity and lipid metabolism disorders, reduce the incidence of non-alcoholic fatty liver disease(NAFLD), protect endothelial cells, prevent coronary heart disease, and delay the progression of osteoporosis(OP). MOTS-c plays a positive role in improving the adverse outcome of a variety of metabolic diseases. This paper mainly reviews the function and mechanism of MOTS-c in metabolic-related diseases.

    查看更多>>摘要:微RNA(microRNA,miRNA)是一类基因表达的负性调控因子,已经成为重要的非编码RNA,调节细胞生理及病理状况下的反应。越来越多的文献强调miRNA在胰岛β细胞遗传、发育、代谢和炎症应激中的重要作用。β细胞是胰岛中的一种内分泌细胞,其主要功能是合成和分泌胰岛素,维持血糖水平的稳定。目前β细胞去分化被认为是糖尿病发病的另外一种机制,β细胞的功能衰竭与β细胞去分化密切相关。在这篇综述中,我们旨在总结调控β细胞去分化的几种重要的miRNA以及最新研究成果。 MicroRNA(miRNA), a class of negative regulators of gene expression, have become important non-coding RNA that regulate the response of cells under physiological and pathological conditions. More and more literature emphasizes the important role of miRNA in the inheritance, development, metabolism and inflammatory stress of pancreatic islet β cell. β cell is a kind of endocrine cell in the islet, its main function is to synthesize and secrete insulin and maintain the stability of blood sugar level. At present, β cell dedifferentiation is considered to be another mechanism of diabetes, and β cell failure is closely related to β cell dedifferentiation. In this review, we aim to summarize several important miRNAs that regulate β cell dedifferentiation and the latest research results.

    查看更多>>摘要:糖尿病(diabetes mellitus)是一种以高血糖为特征的慢性疾病,其患病率在全球范围内不断上升。心血管病变是糖尿病最主要的并发症之一,具有治疗难度大、病情进展快及死亡率高的特征。血管内皮功能受损是诱发糖尿病心血管并发症的始动环节,其持续存在可以作为评估心血管并发症发生风险的一个重要指标。本文综述了糖尿病患者血管内皮功能损伤的机制及无创评估方式研究进展,以期在糖尿病患者中,早期发现并及时干预内皮功能损伤,降低心血管事件的发生风险。 Diabetes mellitus (DM) is a chronic disease characterized by hyperglycemia, the prevalence of which is increasing worldwide. Cardiovascular disease is one of the main complication of DM, which is characterized by high treatment difficulty, rapid progression, and high mortality. Vascular endothelial dysfunction is the initiating link in the development of diabetic cardiovascular complications, and its persistence can be an important indicator in assessing the risk of vascular complications. This article reviews the research progress on the mechanism and noninvasive assessment of vascular endothelial dysfunction in patients with DM, with the aim of early detection and timely intervention of endothelial function impairment to reduce the risk of cardiovascular events in patients with DM.

However, the clinical translation of oral insulin has been hindered by the physicochemical properties of insulin itself and the gastrointestinal barrier. With the application of new materials and technological advancements, novel drug delivery systems have been used for oral insulin administration and have made significant progress. The article reviews the physiological significance and advantages, challenges, drug delivery system, research status, and future prospects of oral insulin.
